Your first visit to Saye, Gette & Diamond Dermatology Associates involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You’ll find all the practical information you need, such as a map and directions to our office, practice hours, office policies and more. We do want you to arrive 30 minutes prior to your appointment time, so you can be ready to see the provider at the start of the actual appointment.

There’s also background information about our committed staff and our first visit procedures. You can also save some time by reading over the patient forms in advance of your appointment. These will all be electronically signed when you come to the office.

Return Visit

On each visit to our office, please be prepared to give us a brief health-related history as well as a list of your medications and allergies. It is very important for us to know as much about your health as possible. All insurance information should also be presented at this time.
It is our policy to follow all federal and state laws and reporting requirements regarding identity theft. Please be informed that it is a requirement that you bring a photo ID used by a local, state or government agency (valid driver’s license, permanent resident/green card, passport, employee or student ID card) to your appointment.

If another provider has sent you to our office, be sure to inform us so that a letter regarding our evaluation of your medical condition can be sent to the referring physician.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
